Congregate Connect serves a variety of populations with housing for low-income adults with intellectual and developmental disabilities. We partner with communities and make a long-term commitment to help them succeed, resulting in positive, measurable outcomes for residents and neighborhoods.
With a portfolio of 8 homes and growing every day, Congregate Connect has honed our skills of negotiation, acquisition, financing, design, building and property management. Congregate Connect's Adult Residential Facilities are adapted for three to four individuals with similar needs and services to live together in a family environment. With the closures of California’s State Developmental institutions, which has taken place in stages since 1996, there has been a shift from institutional housing to small community-based care homes for people with I/DD.
Over years of acquiring and modifying homes to suit the needs of our residents, Congregate Connect has developed proficiencies in all areas of the home improvement process. Congregate Connect is a California housing partner providing specialized solutions in the shift from institutional settings to community-based housing for individuals with intellectual and developmental disabilities. With the historic closure of Agnew’s State Hospital in March of 2009, the need became clear.
Congregate Connect owns and manages a portfolio of 8 Adult Residential Facilities. These different models of housing, designed specifically for residents’ unique needs, provide care on a 1:1 ratio and supervision 24 hours a day, 7 days a week.
We work closely with the California Department of Developmental Services Community Placement Plan (CPP) and the SLARC Regional Center to transition people to our community-based licensed care homes.
Community Care Facilities (CCFs) are licensed by the Community Care Licensing Division of the State Department of Social Services to provide 24-hour non-medical residential care to children and adults with developmental disabilities who are in need of personal services, supervision, and/or assistance essential for self-protection or sustaining the activities of daily living. Based upon the types of services provided and the persons served, each CCF vendored by a regional center is designated one of the following service levels:
SERVICE LEVEL 1 Limited care and supervision for persons with self-care skills and no behavior problems.
SERVICE LEVEL 2 Care, supervision, and incidental training for persons with some self-care skills and no major behavior problems.
SERVICE LEVEL 3 Care, supervision, and ongoing training for persons with significant deficits in self-help skills, and/or some limitations in physical coordination and mobility, and/or disruptive or self-injurious behavior.
SERVICE LEVEL 4 Care, supervision, and professionally supervised training for persons with deficits in self-help skills, and/or severe impairment in physical coordination and mobility, and/or severely disruptive or self-injurious behavior. Service Level 4 is subdivided into Levels 4A through 4I, in which staffing levels are increased to correspond to the escalating severity of disability levels.
